Stop work immediately and ensure the safety of yourself and other personnel, informing others to stop their operations.
Assess the situation, evaluating the nature and extent of the safety issue and evacuate the area if necessary.
Notify the relevant personnel i.e. deputy site manger, HSE, authorised engineer. Communicate the issue clearly, informing them of the issue, the location and what is affected.
Document the incident and report it. Record details of the safety issue, time, location and circumstances surrounding.
Await instruction from relevant notified person.
