In a maximally-compensated metabolic acidosis (which takes about 12-24 hours), Winter’s formula applies:

Potential indications for bicarbonate therapy in metabolic acidosis
Compensation in respiratory acidosis
acute:
inc in HCO3 by 1 mEq/L for every inc in PCO2 by 10 mmHg
chronic:
inc in HCO3 by 3.5 mEq/L for every inc in PCO2 by 10 mmHg
with near-normal pH
How to determine if respiratory acidosis is acute or chronic?
Ratio of change in pH and change in PCO2

>0.8: concomitant metabolic acidosis

Discuss respiratory alkalosis

- bicarbonaturia and dec in acid excretion requires 6 to 72 hours to develop fully
- at at least 1 week to normalize pH
Albumin and metabolic acidosis
Hypoalbuminemia may mask the presence of a high AG metabolic acidosis (HAGMA).
For every drop in the albumin level by 1 g/dL, the normal AG range should be lowered by approximately 2.5
Compensation in metabolic alkalosis
As a guideline, PCO2 in patients with significant metabolic alkalosis should rise by 0.7 mm Hg for each 1 mEq increase in HCO3
The PCO2 also rarely rises above 55 mm Hg in compensation for metabolic alkalosis
