3. No opioids if active SUD (strong)
4. Treat psychiatric issues first (weak)
5. Hx of SUD also avoid opioids (weak)
6. Long-term use - less than 90 MED (strong)
7. and if new no more than 50 MED (weak)
8. Opioid switching (weak)
9. Taper to 90 MED/ lowers effective dose (weak)
10. Multidisciplinary program (strong)
