ACEi Beta-blocker Spironolactone Hydralazine \+ Digoxin Cardiac resynchronisation therapy
Angina
Start with Beta-blocker or CCB then INR Ivabradine - 2nd Nicorandil - 3rd or long-acting nitrates - 1st Ranolazine - 4th
Hypertension with DM
Start with ACEi regardless of age
Hypertension
<55 - A, >55 or afra-carribean C
then A+C
then A+C+D (D=indapmide)
then if K+ <4.5 - spironolactone, if K+>4.5 - alpha-blocker or beta-blocker
HAS BLED
Hypertension Abnormal renal and liver function Stroke Bleeding Labile INR Elderly Drugs or alcohol
Target BP
Below age of 80, <140/90 clinic, < 135/85 mmHg home
Above 80, < 150/90
CKD and hypertension and ACR 70 or more- <130/80 mmHg
CHA2 DS2 VAS
