On every sea going ship on which more than 5 workers are employed, the master must record the appointment in the official logbook.

What are the five stages of a risk assessment?
- Identify the hazards.
- Decide who might be harmed and how.
- Evaluate the risks and decide on precautions.
- Record your findings and implement them.
- Review your risk assessment and update if necessary
What is permit to work?
A formal record to confirm that a risk assessment has been conducted and task specific control measures have been implemented.
What is a safety officers duties?
- Safety inductions of new crew
- Set an example
- Develop high level of safety consciousness on board
- Approach individuals where unsafe practice is observed
- Look for potential hazards and rectify
- Record all accidents and incident
- Investigate all incidents and accidents
- Duty to stop dangerous work
What is an enclosed space?
- Any enclosed or confined space which is foreseeable that the atmosphere contains dangerous or flammable gases
- Has limited openings for entry and exit
- Has inadequate ventilation
- Not designed for continuous worker occupation

Can the work be completed with out entering the space?

What is a level 1 risk assessment?
- Generic Risk assessment
- Issued by company carried out to a high level with knowledge and experienced personal
- Contained with in the SMS. I.e hot work, confined space, going aloft
What is risk assessment level 2?
Task Based:
- Vessel and task specific
- Must be carried out onboard the vessel
- Incorporate risks that may occur during condition specific for the moment the work will be performed
- Carried out by competent person or persons and seafarers who will be involved who understand the work being assessed.
What is risk assessment level 3?
Tool box talk:
- Different type of risk assessment that supports the task based risk assessment.
- Talk through the procedure with seafarers involved prior to starting the job
What is a risk assessment level 4?
Dynamic risk assessment.
What is the difference between a hazard and a risk?
- Hazard is a source of personal injury, harm, or damage
- Risk has two elements- Likelihood that harm or damage may occur and potential severity of the harm or damage
What legislation is COSWP developed under?
The merchant shipping (health and safety at work) regulations 1997
What tasks would you need a permit to work for?
- Working aloft
- Working over the side
- Hot Work
- Working on deck in adverse weather

- Entering an enclosed or dangerous space
What are the Principles applying to a
Permit to Work as stated in CoSWP?
- Should be relevant and as accurate as possible
- Should specify the period of its validity (which should not exceed 24 hours)
- Only the work specified on the permit should be undertaken.
- Before signing the permit, the authorised officer should ensure that all measures specified as necessary have in fact been taken, or procedures are in place.
- The authorised officer retains responsibility for the work until they have either closed the permit or formally transferred it to another authorised officer
- The competent person responsible for carrying out the specified work should countersign the permit to indicate their understanding of the safety precautions to be observed.
- On completion of the work, the competent person should notify the authorised officer and get the permit closed.
- The competent person carrying out the specified work should not be the same person as the authorised officer.
What is a sufficient oxygen level for humans?
The normal level of oxygen in the atmosphere is 20.8%. Any variation from that may indicate a problem and should be investigated further.
