Normal decent of testes
Transabdominal: gubernaculum swelling and hormone control
Inguinoscrotal testosterone is crucial.
Completed in majority by 33 weeks gestation
UDT etiology
30% of premature babies
Testis are intrinsically abnormal
Hormonal lack of testosterone surge to help descent
Mechanical: asso with prune belly syndrome.
Complications of UDT
Malignancy 5-10X more likely Infertility Inguinal hernia: patent processes vaginalis Testicular torsion Trauma
Definitions of UDC, retractile testis and ectopic.
Retractile= normally descended testis which are displaced upwards via cremaster muscle spasm. Therefore can always be put back into scrotum.
UDT= arrested along normal descent path.
Ectopic= has passed through inguinal canal and then has deviated fro, normal line of descent to lie outside scrotum.
