Complications

A

Hemorrhage- uncontrolled bleeding, immediate intervention, blood loss
Infection- red, swelling, tender require contro;
Dehiensece- forced incison , wall sepreation - complete sepreation of opening- wound care
Eviseration- organ potrrsuion through opening, require sugery
Firstual- abnormal drainage connection

Purpose for dressings

A
  • Protection, spliting incision, promote healing, thermal insulation, moist environment, vasular supply
  • May not be required when drainage stops, healing occurs, staples secured
  • Contact layer protect, absorbant layer to collect saturation and protective layer to splint wound.
  • Saline to incision
  • Homeostasis, debridment of wound, protect client, allow granulation tissue and epithelial cells

Principles of wound management

A
  • Debridment to allow helaing, moisture balance, infection and wound protection
  • Absorbant, moisture, breathable, protective dressing barrier

Wet-dry and wet-moist
Hydrogel, contact layers
Moist ( less infection, quick healing)
Dry ( scab, delay healing, scaring)

Dressing aspesis

A

Sterile object remains sterile when touched by another sterile –> sterile object in field, broken, moisture, air, gravity, 1 inch margins contamintion of field.
- Date written on package
- Sterile always above wasit level
- In sight always- never turn back on field
- Doubt sterility
- Check breaks, moisture, expiration dates
- Sterile only with sterile and attendened
- Prolonged air ( unsterile) and wet become contaminated.
- Do not reach across, foreps down, avoid droplet over wound, avoid opening across wound, limit air movement and objects passing wound.

7 Surgical Aspeptic principles

A
  1. Sterile to Sterile ( become contaminated if sterile–> non sterile)
  2. Sterile fields- drape moist/broken
  3. Above waist- is sterile
  4. In sight - must be in sight , never turn back
  5. 1 inch field- around field unsterile
  6. Gravity-flow- fluids contact , instruments held down
  7. Doubt=dirty

Wound sterility

A
  • Clean from least to most contaminated–> clean to dirty
  • Incison line ( least to most, incison line to surrounding skin)
  • Drains ( cleaned afterwards, outward cicles–> clean to irty, least to most contaminated– new gauez)
  • Outward circular motion for drains
  • New gauze to wet and dry incison
  • Avoid bottle , package opening across wound ( Date and time opened) sterile 24hrs
  • Package integrity, intact, breaks/moisture, date, keep within field.
