0 = No response/absent; always abnormal.

1+ = Slight/diminished reflex, present but depressed, low normal

May or may not be abnormal

2+ = Normal, typical reflex

Grade 3+

Answer 4

3+ = Brisk reflex, possibly but not necessarily abnormal

May or may not be abnormal

4+ = Very brisk reflex, abnormal, clonus

always abnormal
