What is a pleural empyema?
A collection of pus in the pleural space.
Causes
- Iatrogenic.
Symptoms
Recurrent fever, productive cough, pleuritic chest pain.
Signs
Dullness to percussion, decreased breath sounds and decreased vocal resonance.
Investigations
CXR, WBC, CRP, Thoracentesis, ultrasounds

Will show a persisting pleural effusion D shape

Thoracentesis
Frank pus in Thoracentesis.
Mortality of empyema
20%
Treatment of empyema
IV Broad spectrum antibiotic (amoxicillin) then oral co-amoxiclav for 5 weeks.
Chest tube drainage.
Supportive care.
