Incompetent lower esophageal sphincter
Delayed gastric emptying
GERD signs/symptoms
Retrosternal Burning
Bitter taste in mouth
Belching, hiccups, dysphasia
Excessive salivation
Frequently occurs at night and or in recumbent position
Maybe relieved by sitting up, antacids, water or food
GERD diagnostics
Consider a referral for EGD: rollout cancer, Barrett’s esophagus, peptic ulcer disease
GERD Management
NonPharmacologic Elevated head of bed Avoid alcohol, caffeine, spices, peppermint Stop smoking Weight loss if obese
Antacids as needed
H2 blockers in high doses at night or divided twice a day dosing
Proton pump inhibitor if H2 blockers are ineffective
G.I./surgical consult prn
