- Heart failure (highest indication)
- Complicated IE - heart block, anular or aortic abscess, penetrating lesions
- Highly resistant organism/fungal
- Persistent bacteremia or fever >7/7 after appropriate therapy
- Relapsing IE
- Recurrent emboli or persistent vegetation
- Vegetation >10mm
- Right sided IE with fevers/bacteremia >7/7 post appropriate treatment
