A

SPIKES

Setting - prepare the environment, private, quiet place to talk, ensure patient’s family support system is in place
Perception - Ask what they know about the condition and their expectations and preparation for bad news
Invitation - Ask how much info they want before proceeding and respect their autonomy
Knowledge - Warning shot of bad news, use small simple chunks of language and check for comprehension
Emotions - Anticipate and prepare for emotional reactions and respond with empathy, acknowledge their emotions
Strategy and Summary - develop a future plan, including treatment plans and referrals. Arrange a follow up meeting
