co-produces with the parents and child or young person. it decides the additional specialist support your child requires in order to meet their outcomes. EHCP will assess and consider your child’s education health and care.
define fair access to care
fair access to care was introduced in 2003 to provide local authorities with a frame work to ensure whether people are eligible. the framework also looked at the risk to see if prevention wasn’t provided.
define individual budgets
people are assessed and the money that they receive is to be used to meet the outcomes outlined in their support plan. the service user would usually self assess so they don’t have to repeat the process.

define choice of residence
means that the individuals should have choice about where and how they live. the authority must make sure the homes can be assessed by the individual who lives there. there should be a range of specialist housing available with options meeting individuals needs.
define housing adaptations.
this could be to an individuals home or an already adapted purpose build home. for example, wheelchair friendly homes would need a lower kitchen sink and bench area. the hob/oven needs to be accessible to a hoist. system might be needed to use the bathroom or get into bed.
meeting housing needs.
care acts removal of geographical barriers
in 2014 that care act removed barriers to provide community of care. this means that someone receiving a particular type of care in one area should still receive that care if they chose to move to another area. there should be no gaps in care and support when a person choses to move. the act describes the processes that need to be followed to ensure their needs are met when they move to a new area.
define decentralisation and commissioning
the process of redistributing or shifting functions from a central authority or location. commissioning is the process of planning, agreeing and monitoring services.
