VENOUS CHAMBER
62.5 mg = med in 50 mL and flush with 50mL = 30 min
125 mg = med in 100 mL and flush with 100 mL = 60 min
Indication: Anemia, Low Iron sat
P/C: currently on antibiotics (WBC eat away iron), if not on anticoagulant = admin 45-1.25m before tx end
Cathflo/Alteplase
LUMENS
+ inject 2.2 mL of STERILE WATER into vial and swirl (too cloudy = bad)
+ 1mg/mL for Alumen and Vlumen
+ (1.X lumen size) X + 0.2 mL of NS
-should have 4 syringes in total!
Indication: interdialytic lock, CVC malfunction (occlusion of lumen)
P/C: evidence of internal or external bleeding
Sodium Citrate 4%

Indication: interdialytic lock, maintains patency of lumens
Heparin
LUMENS
+ 1mg/mL for Alumen and Vlumen
+ (1.X lumen size) XmL of NS
-should have 4 syringes
Indication: interdialytic lock, maintains patency of lumens
C/P: active/suspected internal or external bleeding
Vancomycin
VENOUS CHAMBER
500mg = 10 mL STERILE WATER in vial = 100 mL bag
1000 mg = 20 mL STERILE WATER in vial = 250 mL bag
1 hour before end of tx
Indication: systemic infection
P/C: red man syndrome if admin too fast (itching upper body), dialyzable, check vanco levels and scale prior to admin
Cefazolin/Ancef
VENOUS CHAMBER
500mg - 1000mg = 10mL STERILE WATER = 50 mL bag
15-30 min before end of tx
Indication: systemic infection, HD catheter related infection (gram+ bacteremia)
P/C: allergy, dialyzable
Tobramycin
VENOUS CHAMBER
-withdraw ordered amount + 50mL bag
30 min before end of tx
Indication: systemic infection, CVC related infection (gram- bacteremia)
P/C: allergy, dialyzable, if dose is 120mg+ call md to reassess (higher chance of ototoxicity and nephrotoxicity)
Dalteparin/Fragmin
ARTERIAL PORT
Indication: anticoagulant, low molecular weight heparin
P/C: active internal/external bleeding, HIT (heparin induced thrombocytopenia)
Danaparoid
ARTERIAL PORT
Indication: anticoagulant, allergic to heparin or HIT+ pts
P/C: active or suspected internal/external bleeding
Eprex/Epo
VENOUS PORT
-single pre-filled syringe
Indication: anemia
P/C: chemo tx, hemoglobin +120g/L, uncontrolled hypertension, RBC aplasia
Sodium Phosphate Enema
IN ACID
Indication: hypophosphatemia
P/C: hold is greater than 1.2 mmol/L
