P/C: currently on antibiotics (WBC eat away iron), if not on anticoagulant = admin 45-1.25m before tx end

P/C: evidence of internal or external bleeding

IN ACID

Indication: hypophosphatemia
- if phophate levels lower than 0.6 mmol/L call md to prescribe
- ensure nephrologist schedules blood work for phosphate level monitoring x3/week
P/C: hold is greater than 1.2 mmol/L
