Uterine Artery Embolization (UAE)

A
  • Minimally invasive surgical procedure during in which the uterine arteries are occluded through a sclerosing solution or hemostatic gelatin
  • Also known as: uterine fibroid embolization
  • Treats adenomyosis, uterine fibroid, obstetric hemorrhage caused by placental disorders
  • C/I - not for women who wish to achieve future pregnancy

  • 1-2 hours before (except clindamycin and vancomycin which need more time)
  • Continued no longer than 48 hours postop

  • Cardiac rhythm most critical parameter to monitor = arrhythmias and cardiovascular collapse
  • Urinary output is a secondary concern in management

Uterine Atony

A
  • Failure of myometrial cells to adequately contract in response to endogenous (or exogenous) oxytocin
  • More likely in multigravidas, multiple gestation, hydramnios (excessive amniotic fluid), uterine infection
  • Preventative: IV oxytocin and fundal massage
  • If no improvement = 10 units oxytocin injected into uterus
