A

One review on ½ million women for 8 years (mean age 21 yo) - saw 6999 first suicide effects, and 71 suicide

Double that of non OCP - and tripel for risk of suicide
Risk increased within 2 months

How can OCP cause post pill acne?

A

Post pill acne - 6-12 months after OCP (suppresses sebum) - continues even after pill is stopped (especially when individual comes off OCP)

Gut disruption - to flora (can also contribute to post-pill acne)

Liver health (also can contribute to post pill acne)

Nutrient deficiencies

Endocrinological low [rphesyerpme. Espyrpehm, increased eostrogen)

What are pill bleeds?

A
  • Does not correlate to proper menstruation!!
  • Having the occasional pill bleed is necessary to prevent breakthrough but does not mean you have ovulation

Give overview of progestin use in OCPs

A
  • Progestin drugs supposedly similar to progesterone - but
  • Used in many OCPs, IUD
    Effects thyroid, skin, hair health ect etc etc
  • Particularly type of progestin linked to depression, hair loss, ,,… this is actually more similarly structure to testosterone than progesterone!
  • Progestin only pills - things uterine lining and impairs fluid which also suppressing estrogen - when it was first tested in 1956 it had so many side effects that estrogen had to be added to make it more tolerable!!!!

Why is hair loss common in OCP use?

A

Some of the progestins have a high androgen index / structural similarity to testosterones
Shrinking hair follicles
Some may not have high androgen index BUT may cause rebound hair loss

A

Give overview of marina IUDs effects

A

Small amounts of progestin released into uterus
Thickens lining and impairs cervical fluid
Suppresses approx 85% cycles in first year but then only about 15% in years after
Can reduce blood flow by about 90% - often used hen bleeding an issue (but need to understand tradeoffs - we have much better ways to regulate bleeding!!!!)
morena can allow cycles but suppress bleeding

Give overview of flax seed use cases

A

10g ground flaxseed sig increased the 2:16 ratio in postmenopausal women (when taken for 2 cycles) - favoured 2OH pathway

A lot of flax seed studies are flawed - placebos are not really placebos!!!

Phytogeostrogens = sig higher endometrial thickness, clinical pregnancy and serum progesterone sig higher when phytoestrogens added in days 1-12 follicular stage

Reduced risk of uterine fibroids (by increasing ligand)

  • increased risk of being prescribed uneneccesary
  • increased risk suicide

A
  • flat lines hormones = not having follicle development = decr. GnRH pulses, dear LH and FSH, dear follicle dev, follicle cells = lo LH surge, no oestrogen surge, and FSH surge = no ovulation

might be slightly less effective than combined OCP - as breakthrough bleeds have a sightly higher risk of preg

A
  • synthetic oestrogen affects milk production = cant take OCP

BUT can have migraines, headaches, risk Prost partam depression
