What would you order and avoid for chest pain <35yo?

A

Order:
- ECG (r/o Cardiac causes)
- CXR (r/o Pulmonary/Structural causes)

Avoid:
- PT/PTT/INR (evaluate coagulopathy and rarely assist in evaluating CP)

CP under 35yo is rarely ischemic so we are evaluating for more likely causes such as PE, PTX, myocarditis)

What would you order and avoid with chest pain >35yo?

A

Order:
- **ECG, CXR*
- CBC (eval for infection, anemia)
- BMP (eval electrolytes, renal fx, and metabolic causes)
- Troponin

Avoid:
- PT/PTT/INR

What would you order and avoid for suspected pulmonary embolism for a non-pregnant pt?

A

Order d-dimer if CMT pretest scoreis 0-5
- Age <50
– >500ng/mL
- Age >50
Age adjusted threshold (reference table in image)

Order CT-PE if CMT pretest score is >5
