Q

bolster dressing indicatins

aka tie over

A
  1. mobile or convex areas e.g. face, scalp.
  2. irregular or uneven surface
  3. areas difficult to dress e.g. nose, ear, perineum, axilla.
  4. hematoma or seroma formation is likely

Q

Skin graft indications

A
  1. Cover large wounds where
    primary closure is not possible
  2. Burns (deep second or third degree)
  3. Traumatic skin loss
  4. After excision of tumors or chronic
    ulcers
  5. Post-infection defects (necrotizing fasciitis, etc.)

Q

Contraindications of skin graft

A
  1. Poor vascular bed (exposed
    bone/tendon without
    periosteum/paratenon)
  2. Active infection or contaminated
    wound
  3. Irradiated or ischemic tissue

no bed no blood just bugs

Q

Requirements for Graft Survival

A
  1. Healthy, well-vascularized
    bed
  2. Good contact between
    graft and bed (no
    hematoma/seroma
    )
  3. Immobilization (secure
    dressing)
  4. Aseptic technique

healthy, clean, touching but no moving

Q

Complications of skin grafting

A
  1. Partial or total graft loss (due to infection, hematoma,
    movement ,poor bed etc)
  2. Contracture (especially with STSG)
  3. Poor color/texture match
  4. Donor site complications (pain, infection, delayed
    healing..)

Q

Flap Indications include:

A
  1. Coverage of exposed vital structures
  2. Need for functional reconstruction
  3. Need for aesthetic reconstruction
  4. Coverage of poorly vascularized or irradiated beds
  5. Dead space obliteration Pressure sores, trauma cavities, or after tumor excision

Q

Flap Complications

A

Vascular
* Venous congestion (most common) → bluish, swollen flap
* Arterial insufficiency → pale, cold, no capillary refill
* Partial or total flap necrosis

Infection, hematoma and seroma

Donor Site Morbidity
* Scarring,
* contour deformity,
* loss of function,
* delayed healing

Functional or Aesthetic Problems
* Bulky flap,
* color/texture mismatch.
