Postoperative fever in a 45 years old woman who had undergone laparoscopic cholecystectomy yesterday.
How will you assess this patient
- Charts - vitals charts

- Do physical examination
- Consider management procedures
50 years old man, who underwent knee replacement surgery two days, back is now confused, disoriented and difficult to restrain.
How will you assess his condition.
- Ensure that patient is not a danger to himself or others.
- Put him on high flow oxygen and attach a pulse oximeter
- Ensure IV access through IV cannula
- Take blood samples
- Check all the charts, case notes, nursing records, any previous investigation reports
- Perform focused physical examination on the patient and take history from the family/relatives and nursing staff

- All the vital signs
- Cardiovascular and Respiratory system
- Abdominal system
- Evidence of sepsis
- Evidence of VTE in the legs and chest
- Neuological deficits
What are the causes of post operative confusion?
And what investigations will you do to determine them?
- Hypoxia (most common) - ABG
- CXray
- ECG - Hypotension - meaure vitals
- Infection/sepsis - FBE, Blood C/S, Urine MCS, Chest xray
- Electrolyte imbalance - Serum UEC, BSL, arterial pH
- Cardiac (MI or Arrhythmia) - ECG
- Neurological - physical examination
- Inadequate pain management - see medication chart
- Drug or Alcohol withdrawal (VERY common) - history from friends and family
- Opiate overdose - see medication chart
- Pre-existing thyroid conditions - TFT
Causes of Post operative SOB
- Pulmonary atelectasis
- Pneumonia
- Blood transfusion reaction
- Pulmonary edema due to heart failure
- Pulmonary embolism
- Exacerbation of asthma or COPD
Management of Post Operative SOB due to Pulmonary Atelectasis
- Reassurance
- Call my registrar
- Investigations - FBE with ESR and CRP
- UEC
- ABG
- Blood culture
- Chest xray
- ECG - Refer to the chest physiotherapist to show some breathing exercise
- Panadol for the pain and fever
- Attach a pulse oximeter to monitor your SpO2 and give low flow O2 if required
- Take deep breaths
Move around
No smoking - Can go back to work in 1-2 days if you feel well
Post operative patient with just SOB, no other findings.
What is the diagnosis?
Condition- Pulmonary atelectasis
Cause - 1. Inadequate pain management
2. Past history of smoking
3. Past history of lung diseases
4. Long duration of surgery
Commonality - Very common complication after surgery
