A
  • how far open the cervix is
  • 0 to 10 cm dilated

define cervical effacement?

A
  • how thick or thin the cervical lining is
  • the thicker, the less dilated, birth is not as close
  • thin? pt ready to push

how is cervical effacement documented?

A
  • 0% to 100%
  • 0% means NOT at all thin, very thick yet
  • 50% means halfway there
  • 100% means no thickness, very thin

what does fetal station mean?

A
  • how many cm above or below the ischial spine the fetus is

how is fetal station documented?
0 is the number for the ischial spines - above (or the wider part of the pelvis) is 4 cm long and is said in negatives: ex- -3 fetal station - the more narrow part of the pelvis after the ischial spines i also 4 cm long but is documented in positives "positive 2 fetal station"

what is the amniotic membrane or the "bag of waters" (BOW)
- amniotic sack or the fluid referenced when someone says my water has broke.

define BOW - I
- intact bag of waters - sack is not broken

what does SROM mean?
- spontaneous rupture of amniotic sack - "my water just broke"

what does AROM mean?
- artificial rupture of ammonitic sack, ruptured with a amnihook and snags the membrane to break

what does TACO refer to when a pt's BOW breaks?
- FIRST IS TO CHECK FETAL HR bc that is a change in state - T = time, when did the bow break? - A = amount: how much fluid was there? - C = color: clear, bloody, green, brown - O = odor: foul smell indicating infection?

what is the normal FHR?
- normal fetal heart rate is 110 - 160

where can you find FHR?
- feel on abdomen for the soft part of the babies spine or back and listen there - can't listen through knees and arms of the baby so the nobbles on the abdomen - typically heard LLQ and RLQ

how do we do a head to toe post partum assessment?
- brain - breast - utuers - bladder - bowel - lochia - episiotomy/perineum - epidural site - extremities - emotional state
