A

Observation nurse can assess a patient mood if their happy distressed or agitated, lucidity also through patient skin if pale clammy if they have any skin condition like a pressure sores or if they short of breath

A

A care plan is a legal document all services users are entitled to a care plan. The care plan sets out the services users care goals and outlines how care should be delivered so that the care goals are achieved. A care plan ensures that holistic needs are met. It also ensures continuity of care.
