Expanding hematomas in which zone of the neck do not require pre-op angio?
Zone II – can be directly approached distally and proximally.
Blunt carotid injury grade I
<25% of lumen is narrowed. Give anti platelet rx and observe with followup CT.
Blunt carotid injury grade II
> 25% of lumen is narrowed with either dissection or intramural hematoma. Surgical repair if accessible; otherwise, give anti platelet rx.
Blunt carotid injury grade III
Pseudo aneurysm; repair with surgery or endovascularly.
Blunt carotid injury grade IV
Occlusion. Repair with surgery if accessible; otherwise, give anticoag.
Blunt carotid injury grade V
Transection. Repair if accessible or ligate.
