• coming to modified duties after an off the job injury
• coming back to full duties after an off the job injury
• coming to full duties after being on modified duties due to off the job injury
• coming back to full duties after chest pains
• If off work or on modified duties an MAF is submitted monthly

• Coming back to full duties after workplace injury
• for every workplace injury resulting in lost time and or medical care
• if returning or not returning to modified duties after workplace injury
• monthly after the first doctors visit
