Reporting Procedures
The following items have to be completed before going off duty:
— employee portion within the online safety incident reporting system
— any witness statements within the online safety incident reporting system
— unit officer shall arrange for a repair estimate (via an M5 ticket); duty apparatus contacted the next business day
— if damage occurred to portable equipment bc of the accident, follow SOP 01.07.01 property accountability and control

damage to any unit not defined as a vehicle accident is considered a non-collision accident (ex: snow chain damage, windshield damage from helmets, portable radios, rocks, etc.)

driver could not have reasonably prevented the crash from occurring
preventable accident
driver had reasonable options available to them that could have prevented the accident.
post-accident alcohol/drug screening procedures and considerations
any vehicle accident resulting in a human fatality or bodily injury requiring immediate transport will require the FRD driver to submit to alcohol and drug testing
administrative investigation
non-criminal investigation conducted for the purpose of documenting the conduct, actions, or performance of an employee and to determine whether such conduct, actions, or performance is in compliance with department policy or orders.
