A
1st = perineal skin tear
2nd = perineal skin & muscle tear
3rd = perineal skin & muscle & bit of lateral anal sphincter tear
4th = full tear through perineal skin, muscle & all the way through anal sphincter

A
obstetricians & midwives can both repair 1st & 2nd degree tear in the room
3rd & 4th done by obstetricians in surgery
*with pain relief like local anaesthetic, entonox & epidural
