Explain goals and rationale specific to pt’s condition
Place pt in comfortable, relaxed position, or other appropriate position and loosen restrictive clothing
Observe and evaluate pt’s natural breathing pattern at rest and with activity to determine appropriate treatment and establish a baseline, voluntary or reflexive cough
If necessary, teach relaxation techniques to relax the mm’s of the upper thorax, neck, and shoulders
Demonstrate the desired pattern
Have pt practice in a variety of positions, if appropriate

Never allow pt to force expiration
Don’t allow excessively prolonged expiration
Do not allow pt to initiate inspiration with accessory mm’s and upper chest
Allow deep breathing ex’s for only 3-4 inspirations and expiration’s to avoid hyperventilation
Never allow pt to suck air in by gasping
Avoid uncontrolled coughing spasms
Aboid forceful coughing with pt’s with a history of CVA or aneurysm
Be sure pt coughs in a relatively upright posture
