Risk assessment completed

• Using SMS checklist and risk matrix
• All potential risks identified

Permit to Work is created to mitigate risks found in the risk assessment. A copy should be kept at the tank entry point and on the Bridge, included is;

• Location
• Purpose of entry
• Time of entry and exit
• Enclosed Space has been properly ventilated we validate this by testing the atmospheric conditions
• Atmospheric conditions (taken at regular intervals)
• Safety equipment at entry; rescue tripod, BA cylinders with umbilical cords, BA board, EEBD’s
• Tank sentry in comms with Bridge (should inform of entry and exit times) and entry team
• PPE worn
• Isolations in place
• PTW briefed to all entrants and signed by all entrants, responsible officer and the authorising officer
• Emergency procedures in place
• Adequate lighting

PTW should not be closed until the tank lid has been confirmed secured

What gases are you testing for on the gas meter?

Answer 3

• Oxygen level
• Flammable gases / vapours
• Hydrogen Sulphide
• Carbon Monoxide

Question 4

What are the required gas readings for enclosed space entry?

Answer 4

• Oxygen must be at least 20.8%
• Flammable gases/vapours must be NIL
• Toxic gases must be <50% of the Workspace Exposure Limit (WEL)

Question 5

What are LEL and LFL? How are they managed for enclosed space entry?

Answer 5

• LEL = Lower Explosion Limit
• LFL = Lower Flammable Limit
• Limits will be defined by authorising officer which will be in line with the SMS
