Hard tissue charting defined
Graphic representation fo teh conditions of the client’s teeth observed on a specific date. Charting any and all hard tissue findings such as: existing restorations, treatment needed, defects/overhangs, missing teeth/impacted, anomalous conditions, occlusion, decay/demineralization, open contacts/Diastemas.
Why do we do it?
Provide a permanent record for long term comparison.
Aid in choosing the appropriate x-rays, instruments and products.
Identify treatment and counseling needed
Evaluate trends
Differentiate restorations completed by previous dentist from restorations completed by current dentist.
Evaluate tissue response near placement of specific restorative materials.

Mirror, explorer, gauze, air/water syringe, patient dental history
Sequence
All teeth must be assessed, go from 1-32, this is done after EO/IO, ging specifics and before periodontal assessment. needs to be updated at every appointment.

How is decay detected
Direct visual, tactile clinical examination, transillumination, diagnosing laser, Radiographic evaluation, evaluation of symptoms described by teh patient.
