Changed must be initialed and dated by patient on the health history form and form must be signed and dated.
For any condition requiring a medical alaert must conduct a full write-up
A new healthy history line should be filled when all signature lines are filled, after 3 years, when significant changes have occurred.
Medical consult needed for the following and defer treatment for the following
Symp hep B,C,D. Cancer, chemotherapy, cochlear implant, hemodialysis, hiv/aids, kidney transplant, leukemia, pacemaker, sickle cell anemia, heart attack, stroke, unstable angina. Active TB, Hermes labialis hypertensive crisis, physician recommendation, advanced active hepatitis.
Prosthetic joint
Evidence that dental procedures are not associated with prosthetic joint implant infections, there is evidence that antibiotics provided before oral care do not prevent prosthetic joint implant infection, there are potential harms of antibiotics including the risk for anaphylaxis, antibiotic resistance, and opportunistic infections, benefits of antibiotic prophylaxis may not exceed the harm for most patients.
