Any injury which results in death within 30 days of the accident
Incident
An occurrence other then an accident, associated with the operation of an aircraft, which effects or could effect the safety of operations
Serious injury
Substantial damage
Damage or failure which adversely affects the structural strength, performance, or flight characteristics of the aircraft, and which would normally require major repair or replacement of the effected component
Serious incident (FFIDIAD)
F- flight control system malfunction or failure
F- failure of any internal turbine engine component that results in the escape of debris other then out the exhaust path
I- inability of any required crew member to perform their duties as a result to injury or illness
D- damage to property, other than the aircraft, estimated to exceed 25k for repair (including materials and labor) or fair market value in the event of a total loss, whichever is less
I- inflight fire
A- aircraft collision in flight
D- damage to helicopter main or tail rotor blades, including ground damage that requires major repair or replacement of the blades

An occurrence associated with the operation of an aircraft which takes place from the time any person boards the aircraft with the intention of flight and all such persons have disembarked, in which any person suffers death or serious injury, or in which the aircraft receives substantial damage.
