An occurrence other then an accident, associated with the operation of an aircraft, which effects or could effect the safety of operations

Serious injury

Answer 3

• Hospitalization for more then 48 hours, commencing within 7 days from which the injury was received
• Severe hemorrhages, nerve, muscle, or tendon damage
• Internal organs
• Fracture of any bone (except simple fractures of the fingers, toes, or nose)
• Burns any second or third degree burns or any burns effecting more then 5% of the body

Substantial damage

Answer 4

Damage or failure which adversely affects the structural strength, performance, or flight characteristics of the aircraft, and which would normally require major repair or replacement of the effected component

Question 5

Serious incident (FFIDIAD)

Answer 5

F- flight control system malfunction or failure
F- failure of any internal turbine engine component that results in the escape of debris other then out the exhaust path
I- inability of any required crew member to perform their duties as a result to injury or illness
D- damage to property, other than the aircraft, estimated to exceed 25k for repair (including materials and labor) or fair market value in the event of a total loss, whichever is less
I- inflight fire
A- aircraft collision in flight
D- damage to helicopter main or tail rotor blades, including ground damage that requires major repair or replacement of the blades

Occurrences requiring immediate notification

1. The operator of the aircraft shall immediately, and by the most expeditious means available, notify the nearest NTSB office when:
   A- an aircraft accident occurs
   I- a serious Incident occurs
   O- an aircraft is Overdue and is believed to have been involved in an accident

Preservation of an aircraft involved in an accident

Answer 7

1. Operator is responsible for preserving any wreckage, mail, cargo, and records
2. Prior to NTSB taking custody, do not move anything unless:
   - remove persons injured or trapped
   - protect wreckage from damage
   - protect from public injury
3. If moving wreckage, make sketches, take notes, pictures of original position of moved objects
4. Operator must retain all records, reports, or documents

Written reports

Answer 8

1. The operator will file a report within:
   - 10 days after accident
   - 7 days if an overdue aircraft is still missing
2. Incidents: immediate notification whenever requested by authorized representatives of the board

An occurrence associated with the operation of an aircraft which takes place from the time any person boards the aircraft with the intention of flight and all such persons have disembarked, in which any person suffers death or serious injury, or in which the aircraft receives substantial damage.
