Describe the purpose of an Accident/Near Miss Investigation? Where can you find the form to be used? And who is to conduct?

• The accident near miss form is used to dig deep into what happened to prevent an injury in the future and eliminate the root cause of the incident.
• Identify the situations and circumstances surrounding an accident or near miss.
• The form can be found in the safety center and front office.
• The supervisor or manager of the partner involved in the accident or injury should complete the investigation. Follow up should be done with the affected partner and anyone who works in the same area.

• Compliance deals with meeting the requirements of state and federal laws.
• You have OSHA compliance- deals with issues of safety training, injury reporting, safety equipment and PPE, etc.
• Environmental compliance-deals with wastewater pretreatment, sludge disposal, air quality. Failing to maintain compliance in either area can result in fines and can result in our facility being shut down.
• Safety compliance- Deals with partner training and partner understanding of our safety program. CSOP’s, fire, electrical, LOTO

• An incident is OSHA recordable if the partner receives medical treatment beyond first aid, is given prescription medication, or the injury results in lost time from work.
• If the partner merely receives a non-prescription medication and no medical procedure is performed, the incident is a first aid incident and is not recordable.

• Doing something unsafe usually doesn’t result in an accident but the more times we allow unsafe actions to occur the wider the base of the pyramid and in turn increases the probability of an injury.
