Steps in Burn Algorithm
8
Step 1
Remove Clothing (if not stuck to the Pt) & other restrictive items such as jewelry from all extremities
Step 2
Leave Blisters intact & cover burns w/ Dry Dressing or clean Sheet
Step 3
Keep Pt Warm
Step 4
Initiate Fluid Resuscitation (Lactated Ringers) for Large Surface Area Burns (>10%)
Ideally TWO IV/IO Access points through Unburned Skin
Step 5
Cardiac Monitor & EtCO2
Step 6
Evaluate for Thermal Burns to Airway
*Airway Management ONLY if unable to Vent or Oxygenate
Step 7
Consider Pain Management or Smoke Inhalation AG
Step 8
Rapid Transport to Burn Center (Banner Main) w/ detailed MIST Report
Indicators of decreasing Airway from Burn/hot inhalation
-Burns around nose or mouth
-Soot in mouth or nose (singed nasal hairs)
-Intraoral burns (burned tongue)
-Intraoral swelling (no stridor)
-Visible pharyngeal edema
-Hoarseness of voice
-Inspiratory stridor
Burn Center Referral Criteria
-Partial thickness greater than 10%
-Face
-Hands
-Feet
-Genitalia
-Perineum
-Major joints
-Electrical burns
-Inhalation burns
-Preexisting medical disorders
Preexisting Medical Disorders (That affect Survivability)
If Cardiac Dysrhythmia Begin
Give Calcium Chloride & Contact Medical Control
