What is a Whipples procedure?

A

Pancreatoduodenectomy - resection of head of pancreas, duodenum, gall bladder, lower portion of stomach and distal hepatic duct.

Requires 3 anastamoses:
Pancreatojejunostomy
Hepaticojejunostomy
Gastrojejunostomy

Preop Whipples considerations?

A

Risk stratisfication, NZRISK, Heidelberg score
CPET testing - AT <10ml/kg/min
Malnutrition
Iron deficiency anaemia
Sarcopenia
Jaundice - coagulopathy - endoscopic stenting not indicated however
Alcohol and smoking
Glucose management - insulinoma
Gastric outlet obstruction
Secretory neuroendocrine tumours - gastrinoma (PPI), insulinoma (diazoxide), glucagonoma (high dose VTE prophylaxis, ocreotide)
