A. Work requiring live line tools shall be performed in accordance with APM rules

B. All live line Tools shall be approved by the safety department and shall have no alterations made to them

Definition. A clearance is a formal authorization officially issued to a qualified person at that person’s request to work on electrical line or piece of operating equipment which is inherently to Hazardous To work on while in service and has been deactivated or deenergized in a prescribed manner and placed in A safe condition to be worked on

This authority carries with it a statement of the status of the line or equipment and is a guarantee that this status will not be changed in any way which might be detrimental to the safety of personnel

A

Rubber gloves.
A. Rubber gloves shall be worn when working on exposed energized conductors rated from 120v to 17kv unless live line tools are being used.

  1. When using rubber gloves insulate/isolate practices shall be used.
  2. When using rubber gloves a qualified person trained in CPR and emergency rescue must be present on the ground.

A

PORTABLE LADDERS.

  1. When working off of ladders ladders shall be secured or tied to prevent slipping or falling.
  2. Latter shall be placed at pitch that is one quarter the length of the ladder at the ground to the point of support.
  3. Ladders shall be equipped with a non slip base.
  4. Metal ladders or wood ladders painted with a pigment based paint shall not be used.
  5. When using ladders to gain access to roof’s landings or underground structures side rails shall extend a minimum distance of 3 feet above the point support
  6. Permanent underground ladders shall not be used if removed from their designated supportive assembly.
  7. Ladders shall not be placed in front of doorways.
  8. Employee shall face ladder when climbing or descending
  9. When working to the side of the latter employees body shall not extend the latter side rails
  10. Employees shall use makeshift substitutes as a ladder
  11. Employees shall not sit, step or climb to the top cap or the step below the top cap without fall protection.

Rule. 141

A

GROUNDING.
A. De-energized and tested de-energized.
B. permanent grounding object

C. Grounds must be grounded to the ground before contacting conductor.
D. Person grounding shall make sure other are away from grounds before contacting conductor.
E. grounds shall be installed where visible for at least one crew member.
F. Grounds shall be installed where accidental contact of unprotected conductor can’t happen.
G. When splicing conductor together with a possible difference in potential on the ground EPZ mat shall be used or a ground shall be place on each side within 10ft of work area. Ends being spliced shall be bonded with jumper.
H. When Grounding in sub station with portable grounds sub station shall provide grounds.
I. Before grounding one circuit on a multi circuit line & circuits are separately bonded, you shall test de-energized circuit and bond to make sure there’s no difference in potential.
J. Bonds shall be grounded before climbing through multiple energized circuits.
K. bond or metal fixtures supporting conductors must be grounded before contact.
L. Substation grounds shall not be considered for approved personal grounds.

BACKFEED

  1. Backfeed shall be eliminated by disconnecting with an opening (opening primary or secondary jumpers, fuses, primary switches, cut outs, etc) Or by grounding and short-circuiting the low or high-voltage side. Or they shall be considered energized.

TESTING OF WOOD POLES.
A. Before climbing pole a visual & sound test shall be made for damage or decay
B. 2 9/16 holes drilled 90 degrees apart. 1st hole 12” below ground 2nd at ground level. Both drilled at 45 degree angle through center of pole but without getting closer then 2” to other side.
C. Holes shall be plugged w/ shell thickness indicator & plugged w/ 5/8” treated wood dowel

Rule 209.

A

WORKING IN ELEVATED POSITION.
A. before climbing familiarize with the circuit, voltages, apparatus, and any hazards.
B. no more then one employee shall ascend or descend a pole at a time.
C. Before climbing any structure employee shall make sure structure is strong enough to hold them.
D. Before removing guys or any pole supports employee in charge shall make sure pole can with stand the change in stress without falling over.
E. employees shall make sure height nail or pole brand is in the correct place before climbing. 25-60’ pole brand at 10’. 65’ & above 13’
F. Employee shall not transfer from bucket to pole
G. Hooks won’t be worn when working in an aerial lift.
H. Neither bucket or boom will be allowed to make contact w/ energized conductor.
I. Any power tools used in the air w/ a path to ground shall be guarded or protected in a way to prevent contact w/ energized conductor.
J. Only approved power saws shall be used on erected poles.

Rule 222.

A

USE OF LIVE LINE TOOLS
A. when using live line tools hands shall not be placed closer than absolutely necessary. And not to exceed 25” on 12kv & 28” on 16 to 33kv.
B. metal parts of hot sticks shall not touch any part of pole or structure.
C. Other work shall not be done when hot stick work is in progress
D. When using hot sticks adequate protective equipment shall be used on primary & low voltage conductors or any other line in reaching distance.

Rule 302.

A

UNDERGROUND STRUCTURE SAFEGUARDS.
A. Whenever an employee enters a vault, manhole, or by vertical means a qualified person shall say at the surface except when:
1. There are no energized cable or equipment and
2. A protective device is placed around and
3 forced ventilation is provided.
B. whenever the cover is removed from an underground structure:
1. Warning devices shall be displayed until cover is back in place
2. Tools and equipment shall be placed to present the Lear hazard to pedestrians and employees in hole
3. Barricades shall be used unless constantly attended.
C. Barriers & safeguards shall be used for walk in vaults.
D. Heat scan before entering structure.
E. Automated Underground switches shall be made local and non-automatic/solid before working in proximity to the switch. And switching center shall be notified.

A

Safety straps and lanyards.
A. Employees shall where wear safety straps and lanyards when working in any elevated position, unless proper railings or guards are used.
- when lanyard is used it shall be at a length as to limit the fall to no more then six feet.
B. employees shall not belt off to adjacent structures while working from an aerial lift.
C. Employees shall be belted at all times when on a wood pole.

Rule 211.

A

Setting or removing poles.
A. Persons not engaged in pole setting shall keep out of work area
B. it’s not allowed to be on gin pole when being used to raise another pole.
C. When setting poles between or near energized lines
1. The shall not be allowed to make contact with lines.
2. A minimum of 3’ shall be kept between unprotected conductors energized below 72 kv
3. Employees on butt shall wear rubber gloves
4. No one shall step on or off or touch truck while pole is being set.

Rule 147

A

Working distances

A. No employee shall get closer than 25” to voltages 600 to 15kv and 28” to 15kv to 36kv

B. Distances may be reduced provided adequate protective devices are used. Such devices shall be applied and removed with approved insulating devices.

C. Contact shall no be made with protective devices on conductors energized above 7500 volt except with live line tools.

Rule 126?

A

Cranes, hoists, derricks, booms, and winches.

A. A crane, boom, hoist, derrick, or winch shall not be loaded beyond the rated capacity or safe working load

B. When using these equipment near energized lines or equipment, employees operating this equipment shall not stand on a grounded surface other than the equipment itself. All other persons shall stay in the clear.

C. Employees shall not ride on loads.

D. Employees shall not move loads over others heads. And loads shall not be left unattended in the air. And employees shall not walk under loads.

E. Employee controlling equipment shall only take signals from one person during operations.

F. Rigging equipment shall be inspected before and after use. Defective equipment shall repaired or discarded immediately.

G. All equipment shall tested after the start of each shift.

H. Equipment shall be positioned so that it doesn’t come closer than 10’ to voltages from 6 to 50 kv

Rule 127.

A

Work area protection and traffic control

A. Approved warning sign, barriers, guards, flags, alternate pathway, and lights at night shall be installed and maintained whenever hazards exists due to: - moving or stationary vehicles - pedestrians - exposed energized parts - open excavations - walk in vaults or open underground structures.

B. Properly instructed flaggers shall be used when when signs and flags aren’t good enough. Flaggers shall be illuminated in the dark hours and red flags can temporarily be used when stop/slow paddles aren’t available.

C. When on foot and exposed to the hazard of vehicular traffic, a traffic vest must be worn. Vests can be taken off when traffic control is established.

D. Flashing amber lights shall be used when Ariel lift truck is in use but they may be turned off when traffic control is set up.

Rule 131.

A

Care and testing of high and low voltage and dielectric testing of high voltage rubber protective equipment.

A. Rubber gloves shall not be worn inside out or without keepers. They shall be exchanged when the become damaged or suspicious of damage. Keepers shall only be worn over rubber gloves.

B. Low voltage Rubber gloves shall be inspected and given the roll and air test at least once a day before use.

C. High voltage rubber gloves shall be given the air and water test everyday before use. They shall also be visually inspected over their entire inter and outer surface for burns, cuts, cracks, or punctures. Also inspect the cuffs.

D. When not in use rubber equipment shall be protected from mechanical or chemical damage and stored in a place with nothing else in it.

E. Rubber blankets and sleeves shall be given a visual roll test before each use.

F. Rubber blankets when used on ground shall be protected from damage and moisture by use of tarps, canvas, or protective mats.

G. Extreme care shall be given to avoid puncture of all protective equipment. To avoid corona and ozone damage cover shall not be left over night on energized lines.

H. Rubber gloves, blankets, and sleeves used on high voltage rubber gloves used on high voltage conductors shall be dielectrically tested every six month. And dated of test day.

I. Low voltage rubber gloves must be dielectrically tested every six months and dated as well.

Rule 316
Automatic circuit recloser operation. (No test) A. automatic circuit recloser on all curcuits in an ug structure when: 1. Testing, grounding, cutting, spiking, repairing cable or components normally energized above 600V.2.Disassembling cable components normally energized above 600V. 3. Installing or removing electrical or communication cable. 4. Relocating energized high-voltage underground cable or equipment. 5. Filtering or replacing oil on energized high-voltage underground equipment. 6. Operating energized loadbreakcomponents. 7. NOTE: In addition to the above requirements supervisors in charge may make automatic circuit reclosersnon-automatic whenever they deem it necessary for the safety of the employees performing the work or may consult with their manager to determine if a line section or piece of equipment should be de-energized

Rule 306.
Working on de-energized cables. A. Cable or equipment being worked on shall be positively identified by tags, duct locations, maps or by approved testing means before work can be started. B. Cable and equipment shall be considered energized and worked with protective devices until it has been tested de-energized and grounded. C. Clearances shall be obtained before grounding. After de-energizing and working on cable energized above 600v all conductors shall be short circuited and grounded. 1. Between the place work is being done and each possible source of supply or. 2. At the work location or. 3. As close as practicable to the source of supply. D. Cable energized normally energized above 600v shall be spiked or grounded at the work location. Spiking shall be done remotely. Spiking shall be done to prove able de-energized. E. All switches that can energize cable that's being worked on shall be opened locked and tagged.

Rule 140.
CHAIN SAWS 1. Employees shall be trained to use chain saws. 2. Only approved chain saws that weigh less than 25 pounds can be used from an elevated position. 3. Chainsaws shall not be attached or suspended from body belt or harness 4. Chainsaws shall be secured by a separate line while working in an elevated position. 5. Chain saws be gripped at all times when being used. 6. The saw engine shall be off when work on the saw.

Rule 206.
DIGGING HOLES 1. Employees shall stay clear of the auger on the pole hole digger. 2. Holes dug in advance of line work shall be covered and anchored down by a covering of dirt or other suitable means.

WORKING POSITION. 1. When working on energized line or apparatus work shall be done from below when possible.

Rule 204.
Testing lines and equipment. -electrical equipment and lines shall always be considered energized unless they are positively known to be de-energized before starting work. Preliminary inspection or test shall be made to determine what conditions exist.

Rule 207.
Energized line - the term energized lines is defined in this rule as any lines energized excess of 300volts. - when work is to be done on or near energized lines all energized conductors, grounded conductors, or guy wire in reach of any part of body shall be covered with protective equipment. - when working on energized lines or equipment, any other energized lines and normally energized equipment in reach shall be covered. - in applying protective equipment an employee shall always protect the nearest and lowest wires first insuring personal protection at all time. When removing cover the reverse order shall be maintained. - work shall not be done on downed overhead lines until they have been proven de-energized and grounded. Unless being cut in the clear with use of live line tools.

Rule 221.
Live line tools - all lines energized above 7500 phase to phase volts shall be handled with live line tools. Unless employee is rubber glove trained. - all live line tools shall be approved by corporate safety and shall have no alterations without approval. - when work is to be done with live line tools on energized lines two qualified employees shall be assigned to do the work.

Rule 230.
Climbers. - climbers shall be worn only when doing work that requires them - climbers shall not be used after worn or filed to less than 1 1/4 inch. Measured on the under side of gaff.

Rule 308.
Installing or removing underground cable. A. When installing or removing cables in proximity is energized exposed conductors above 300 volts adequate precautions shall be made to prevent contract between cable or metallic pulling device. And employees handling cable, reels, or tending dolly shall be protected as well. B. Ducts shall always be fished in the direction which presents the least hazard. Employee shall be at each end unless employee in charge deems there is no hazard C. When pulling cable at riser near energized conductor above 300 volts, approved pulling device shall be used based on cable size and type. D. Pull ropes, fish tapes, and cables shall not be pulled through ducts already occupied with cable.

Rule 130.
Safety straps, life lines, railings, etc. 1. Employees shall use appropriate fall protection as required when working in an elevated position. 2. Employees shall inspect all fall protection before each use. 3. Employees elevated in a boom must be secured with lanyard. 4. Employees shall make sure safety belt is secured in both D rings before before putting body weight into belt. When using safety strap both snaps shall not be placed in the same D ring. 5. Wire hooks shall not be used on body belts. 6. When working over water employees shall wear life jackets when working alone. 7. Employees working in a boatswain shall be belted off to a fixed part of structure being worked on. Boatswain shall be inspected and maintained in a safe condition. 8. No welding, burning, and sand blasting shall be done from a boatswain suspended by a synthetic rope.

Rule 135.
Tools. A. Only tools in good condition shall be used. B. Field fab tools must be approved by management. C. Tools must not left in an elevated position unless protected from falling. D. Metallic rulers shall not be used near exposed energized equipment. E. Hand lines shall be used to raise and lower tools And light weight material. Hand lines must be made with at least 1/2 rope. Non conductive sheaves shall be used on hand lines. F. Powder actuated tools shall only be used by qualified employees and shall be stored in a case.

Rule 216.
Transformers - no work shall be preformed on the high voltage side of the transformer until either: 1. The fuses have been pulled and the taps removed from the line. 2. The leads between the open switches or fuse holders and the transformer have been grounded.

Rule 215.
Capacitors A. Before working on capacitors of any voltage they shall be de-energized with the approved device. In addition one of the following shall apply on voltages above 600. 1. The jumpers on the fuse holders shall removed from the line. After waiting 5 minutes the capacitor terminals shall be short circuited and bonded to the case using live line tools. 2. After waiting 5 minutes the leads between the open fuse holders the capacitors shall be grounded and the capacitor terminals short circuited and bonded to the case using live line tools. High voltage rubber gloves can be used while placing the short circuiting and bonding jumpers across the terminals on voltages up to 7500 volts. On voltages above 7500 volts live line tools must be used. B. Before climbing into or through an area with a capacitor the automatic capacitors shall be de-energized and controls made inoperative. C. Contact shall not be made with pole top apparatus as specified in rule 205 or other metal hardware in the area of an energized capacitor installation.

Rule 303.
Energizing equipment. A. No new piece of cable or switching equipment shall be energized until it has been positively identified by tags or other approved marking procedures. B. No piece of equipment shall be energized until all terminals, cables, and positions have been safe ended, end belled, or otherwise protected in a way that would protect a short circuit, ground, or personal contact. C. Employees shall not enter any structure where new, rebuilt, or modified high voltage cable or equipment has been energized for the first time until either have been energized for a minimum of 5 minutes.

Rule 205.
Pole top apparatus. A. All energized cutouts shall be operated with a switch stick or approved device. B. Contact shall not be made with ungrounded, energized transformers, capacitors, regulators, automatic reclosers, bonds, or other pole top apparatus except when worked with approved devices. Contact can be made to case ground equipment as long as approach distances can be maintained from all exposed energized conductors. C. Contact shall not be made with case grounded parts of pole top equipment when working on energized conductor. D. When working in location of a pole top air break switch excess of 7500v hardware of switch shall be tested de-energized immediately before working in location. E. Automated pole top switches shall be made local and non automatic before working above or in proximity to the switch. Switching center shall be notified whenever the status of the automated switch is changed.

Rule 203.
Bonds. In addition to bond wires, the term bond also refers to metal x-arms, metal insulating pins, space bolts, or anything attached to an insulator holding up an energized conductor. A. When working on a bond of an energized circuit the bond must be grounded or worked on in rubber gloves (only when the insulator is in good condition) or with live line tools. If energized above 20kv it may only be worked with live line tools. Unless conductors are in suspension insulators and the insulator is in good condition. B. Every insulator on the circuit shall be check for any damage before contacting the bond. If the insulator is damaged more than 30% the conductor must be cleared or de-energized before contacting, and if it's intractable to do so bond must be worked with live line tools.

Rule 214.
Wire Stringing A. When stringing or taking down wire, keep the number of people handling the wire at the same time to a minimum. Running lines, hold down lines, and tag lines shall be left attached until secured. The person in charge shall make sure proper safety precautions are made to make sure this work is safe. B. When stringing or taking down wire over or along street or highways, pulling equipment shall be provided with flags on front and rear. Also barriers and flag men shall be used is required. C. When stringing or removing wire above or below energized lines adequate precautions shall be made to insulate the employee from the wire or stringing equipment by use of rubber protective equipment or other approved methods. Employees shall not contact stringing equipment in a way which they're bodies become a parallel path to ground. Conductors being pulled and handled must be grounded. Conductors must be pulled through traveling grounds at the pay out of at the first pole.

Rule 1007.
Heat Stroke. ``` A. Heat stroke is a response to hear. That cause high body temperature that is life threatening. B. Symptoms: - hot dry skin - flushed or red face - fast throbbing pulse - extreme high body temperature - headache - dizziness and nausea C. First aid. - call 911 - move victim to cool shady area. Maintain body temp. - lie down in comfortable position - loose or remove tight clothing - put ice packs on forehead, back of neck, under arm pits and behind knees - only give victim water when body temperature lowers and is conscious D. Prevention - use sun screen - wear long sleeve shirts and pants - find shade when needed - eat light meals and drink lots of water ```

First aid - move victim to cool area - keep victim lying down with with head level - maintain normal body temperature - give sips of water - if symptoms don't improve in 30 minutes or less call 911 ```

Heat cramps. A. Heat cramps are brief painful cramps that occur during activities in a hot environment. B. Symptoms - cramps in the thighs, calves, and abdominals. C. First aid. - move victim to cool shaded area - stretch the cramped muscles to relieve pain - drink water - if symptoms don't improve in 30 minutes call 911
