Mesenteric adenitis

Define mesenteric adenitis.

Answer 1

Inflamed lymph nodes within the mesentery. Can cause similar symptoms to appendicitis and can be difficult to distinguish between the two. Often follows a viral infection and needs no treatment.

How can you diagnose mesenteric adenitis?

• US - enlarged mesenteric lymph nodes OR
• CT abdo pelvis OR
• Larparoscopy (if being done for ?appendicitis) - large lymph nodes may be seen when doing laparoscopy for appendicitis, with a normal appendix.

What are the clinical features of mesenteric adenitis?

Answer 3

• Generalised tenderness throughout abdomen (not localised to RLQ)
• History of recent or current URTI

Differential: appendicitis or non-specific abdominal pain (which is pain which resolves within 24-48 hours, and less severe than appendicitis)
