Q

fluticasone MOA

A

a synthetic trifluorinated corticosteroid with anti-inflammatory effects. It is a human glucocorticoid receptor agonist that inhibits multiple cell types and mediator production or secretion involved in asthma. Fluticasone has anti-inflammatory, antipruritic, and vasoconstrictive properties. Corticosteroids are thought to act by the induction of phospholipase A2-inhibitory proteins, lipocortins. It is postulated that these proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. Arachidonic acid is released from membrane phospholipids by phospholipase A2

Q

fluticasone/umeclidinium/vilanterol MOA

A

Fluticasone furoate provides anti-inflammatory activity via activation of the glucocorticoid response element, inhibiting pro-inflammatory transcription factors, and inhibition of antigen-induced lung eosinophilia. Umeclidinium inhibits the M3 receptor in the smooth muscles of the airway, leading to bronchodilation. Vilanterol increases cyclin adenosine monophosphate levels, which causes the bronchial smooth muscles to relax and inhibition of release of mediators of immediate hypersensitivity from cells

  • Advise patient that drug is not to be used to relieve acute symptoms and extra doses should not be used for that purpose. Instruct patient to treat acute symptoms with a short-acting beta-agonist rescue inhaler
  • Instruct patient not to exceed maximum recommended dose of 1 inhalation/day
  • Advise patient to rinse mouth with water (do not swallow) after use to reduce risk of oropharyngeal candidiasis
  • Encourage patient to avoid abrupt discontinuation - Warn patient to report symptoms of an infection, hypercorticism, or adrenal suppression

- Control of asthma or COPD symptoms, as measured by pulmonary function tests - frequency or asthma or COPD exacerbations - Need for rescue inhaler use
31
fluticasone with salmeterol MOA
Fluticasone is a synthetic trifluorinated corticosteroid with anti-inflammatory effects. It is a human glucocorticoid receptor agonist that inhibits multiple cell types and mediator production or secretion involved in asthma. Salmeterol is a long-acting β2-adrenergic agonist, stimulates intracellular adenylyl cyclase in catalyzing the conversion of adenosine triphosphate (ATP) to cyclic-3',5'-adenosine monophosphate (cyclic AMP). The increased cyclic AMP levels result in the relaxation of bronchial smooth muscle and inhibition of the release of mediators of instantaneous hypersensitivity from mast cells.
