Origin
At jugular foramen as sigmoid sinus continuation (right is usually larger than left)
Course
Descends inside carotid sheath lateral to ICA, CCA, and vagus nerve
Termination
Behind medial end clavicle by joining subclavian vein forming brachiocephalic vein

Line between sternoclavicular joint and point between mastoid process and mandible angle (same surface marking of CCA, ICA, and carotid sheath)
