Online medical direction is provided through radio or telephone connections between the EMT and the medical control facility. Off line medical direction is provided through written protocols, training, and standing orders.

Ambulance Calls You are dispatched to a two‑car motor vehicle collision. On arrival, you see minimal damage to both vehicles because they were traveling less than 25 mph (40 kph) when the collision occurred. You and your partner interview and examine all of the patients and find no apparent injuries. Dispatch contacts you and asks if you need an ALS crew to respond to your location. Explain how you would respond and why.
You would decline the assistance of an ALS ambulance crew. ALS, or advanced life support, means that the crew would have a higher level of training and could perform more advanced patient care procedures than you or your partner could. Because there were no injuries resulting from this motor vehicle collision, there was no reason to summon advanced care.

Ambulance Calls You and your partner are both EMTs and are dispatched to a private residence in response to a report of a hanging. You arrive to find a distraught man in the front yard, who quickly explains that he found his teenage daughter in the garage hanging by the neck from an extension cord. You enter the garage and find the patient unresponsive and not breathing, but with a weak pulse. Using EMT airway skills, neither of you is able to successfully open the patient’s airway enough to provide ventilations. What should you do?
Because none of the EMT level airway skills were successful, it would be critical to request or rendezvous with a provider capable of using advanced airway techniques. You would contact dispatch and arrange to have an ALS crew either meet you at the patient’s location or load the patient and meet up with an ALS crew while en route to the hospital.
